i have ccm 4.2
i modified ringlist.xml and distictivelist.xml (whatever the name was) with
a new ring raw file and a name following the rule defined in the cisco doc.
i reset the tftp server and reset the phone (mine only).
then i went to setting then ringtone. i see only two ringtones and the
following message "tftp error: ringlist.mxl ..." same thing on the debug on
the phone.
after giving up on troubleshooting, i copied the original back to the
location tftp folder.
then tftp reset and then phone reset.
i still get the error message.

any idea?

check the ntfs file permissions on your files on the cm server

turn tftp traces up to detailed, restart TFTP service, and from your PC
execute this command:

tftp -i <tftp_server_ip> get ringlist.xml

grab the tftp traces from c:\program files\cisco\trace\ctftp (or tftp,
don't reaall just now) and search for either your PC IP or ringlist.xml
to see what happens. there may be several trace files generated.
earlier files will show files from hard disk being loaded into memory,
an important step for your ringlist.xml.

it had to do with the file permission or security setting.

i found it on Puck.net
